The following forms are required upon entrance for registration purposes.

    Please read our Proud History Packet and fill out the agreement form attached.

    Please carefully complete the attached registration form in its entirety. Please mark any boxes that do not apply with "N/A."

    A school photographer comes in twice a year for school pictures. Our teachers and students like to work on art projects regularly and include portrait pictures in them.

Medical Forms

The following forms are would be used to sign in medication for your child.

  • Physical Form

    The following form must be signed and stamped by your child's primary care physician upon enrollment.

    Please fill out Section A for all short-term medication authorizations.

  • Long Term Medication Form

    Medication that needs to be used for more than 10 working days must be signed in using the attached form. Section A of the form must be completed by a parent or guardian first, then section B would be completed by your child's primary care physician.

    Please use the attached form to sign in sunscreen, diaper ointment or creams, and insect repellents for your child.
